Hannah Whitehead is a Clinical Practice Leader with Kaiser Permanente Northern California focusing on regional initiatives to improve serious illness care across 21 medical centers in Northern California. Ms. Whitehead comes to the role with 20 years of experience in healthcare caring for patients with serious illness and is a board-certified Geriatric Nurse Practitioner and Psychiatric Mental Health Nurse practitioner. Hannah completed her undergraduate degree at Cornell University and Masters of Science in Nursing at University of California San Francisco. In her role as Clinical Practice Leader at Kaiser Permanente Northern California, Ms. Whitehead has led the design, testing and implementation of programs to accelerate primary palliative care: (1) ACP education and training for 500 nurses, social workers and physicians and (2) specialty-tailored Serious Illness Communication trainings for nurse practitioners and physicians (3) Primary Palliative Care Catalyst Program.
