San Francisco Palliative Care Workgroup
Loren Pogir is the Executive Director of the Palliative Care Work Group, leading the inaugural San Francisco chapter and the new East Bay chapter. The Work Group brings together health systems, community-based organizations, and government partners to strengthen palliative care access and advance care planning across the region.
She is also the Founder and Managing Partner of Transforming Care Partners, a consulting firm that supports foundations, health systems, health plans, and mission-driven organizations working to improve aging, serious illness, and end-of-life care. Previously, Loren directed a $40M annual Health Care portfolio at the Gordon and Betty Moore Foundation and held senior strategy and operating roles at BlackRock.
She is the author of California’s Palliative Care Evolution and serves as an advisor to the Serious Illness and End-of-Life Funders Group at Grantmakers in Aging and on the board of Thriving in Place. Loren holds a BBA and MPA from The University of Texas at Austin.
